1. Please schedule an appointment on the calendar below for a quick virtual appointment with a member of our intake team. This is at no charge.
1a- During this appointment, our team will address your clinical concerns and go over any questions that you may have.
1b- Any paperwork that is not completed during this appointment will need to be completed prior to the next step.
2. The intake team will speak with our clinical team to determine which clinician would be the best fit for you!
3. Insurance will be verified and you will receive an email from our billing team with this information.
4. Your provider will reach out to schedule your intake appointment!
